Behavior Diary

Hard moments have patterns.

When someone with dementia becomes upset, confused or restless, there's often a reason we can't see in the moment. Log each episode here. After a week or two, the diary shows when and why it tends to happen.

Why track

Understand before you medicate.

Health professionals recommend looking for causes and patterns first — pain, infection, hunger, noise, the time of day — before medicines. A diary turns “she gets upset sometimes” into information you and the doctor can act on. Moderate evidence

Remember

  • A sudden change in behavior can mean infection, pain or delirium — call the doctor.
  • If anyone is in danger, get to safety and call 911.
